How Do I Qualify For Home Health Services?

Your physician will evaluate you and determine what services are appropriate.  The physician will submit the order for treatment to Optimal to begin your services.  This personalized plan may include one or more types of services - nursing care, physical therapy, occupational therapy, speech therapy, or personal care services.  Medical Social Workers are also available to assist patients and their families through social and practical issues; such as coordinating community resources, and financial issues.

Will my insurance cover my services?

Most commercial insurance plans, HMO's, PPO's, Medicare, and Medi-Cal cover home health services.  The Optimal staff will contact your insurance company for you to verify coverage and obtain any needed authorizations.  Your insurance will be billed directly for you and staff will assist you understanding your benefit plan.

How often will Optimal come to my home?

The treatment plan ordered by your physician will determine how often the Optimal staff visits you.

What happens if I need help or have a question?

The home health nurse is available by phone 24 hours a day, 7 days a week to answer questions, assess patient situations, offer support and put your mind at ease.  Patients and their families will always have access to help when they need it.
