RN Case Manager


The RN Case Manager must have a valid California State Board of Nursing license is required. You must have a minimum of one year professional nursing experience with the last three years and pass a pre-employment physician's health clearance, drug screen and a TB test is also required. A current CPR card, valid California driver’s license, and an insured automobile in good operating condition is also required. This position requires managing multiple priorities, good time management skills, assessment skills and organizational skills.A high level of interpersonal skills are required to successfully work with patients, families, caregivers, interdisciplinary team members, and physicians.Supervises LPN/LVN in administration of clinical care to patients.The Case Manager works with patients/families to solve issues effectively as they arise which essential to succeed in this position.The duties include responsibility for timely initiation of the Plan of Care as per agency policy, which incorporates initial evaluation and re-evaluations as the patients condition changes.The education of the patient/family/caregivers along with coordinating all referrals for required services.Continuous review of caseload that ensures appropriate utilization of services and skill level needed, monitoring eligibility status, and reviewing documentation in medical records.
