ImageOptimal Hospice Care uses all available tools to creatively and expertly provide care at the end of life.  Our innovative approach supports the individual needs of patients and their families to maintain quality of life.  At Optimal, we believe everyone should have access to quality care and believe in the importance of delivering what we promise.  

With Optimal, hospice care brings services to the patient’s home, rather than the patient going to a place to receive care.  When surveyed, more than 90 percent of Americans say they would prefer to receive care at home if they knew they had six months or less to live.  Despite these wishes, most people spend their final days in hospitals or settings other than their home.

Hospice helps patients stay in the comfort of home with support for the patient, as well as the caregiver.  Here is a brief overview of how this care works.

Here to Help

Caregivers often wonder, “What do I do if something happens?” Hospice is always available to answer questions, assess patient situations, offer support and put minds at ease.  Hospice nurses are specially trained to handle the situations that arise at the end stages of life, so families can worry less about, “What should we do?” and focus more on enjoying the time they can spend together.

Care at Home

A hospice nurse meets with the patient on a regular basis to provide medical care, discuss changes in the patient’s condition and help resolve problems.  Knowledge is a crucial tool at the end of life, so hospice helps families understand the illness, plan for future changes and know the most beneficial ways to care for their loved one.  This education empowers families with tools for caring and gives them piece of mind knowing they have a trained professional to support them in care.  

Hospice cares, not only for the physical symptoms, but for the whole person, including the emotional, social and spiritual needs.  A team of hospice professionals works with the patient’s nurse to meet these needs and support the family in encouraging and practical ways.

Additional Services

Optimal Hospice assists with the practical needs of a patient by facilitating delivery of medications related to the terminal diagnosis, as well as medical supplies and equipment, to the patient’s homes.  Families can also experience encouragement and rest by utilizing respite care or grief support in the bereavement program.

Optimal Hospice Care is a Medicare certified agency and has been accredited by the Accreditation Commission for Home Care (ACHC).
