Home health care is based on each patient’s individual needs and the plan of care prescribed by their physician, so this care can look very different for each person. The physician may request for several different members of the home health team to provide care to best accomplish the patient’s goals.
These team members include:
Physicians
The patient’s personal physician continues to be responsible for overseeing medical treatment and works with the home health team to provide the highest quality medical care in the patient’s home.
Nurses
Home health nurses visit the patient regularly to provide skilled care and educate the patient and their family members on effective ways to improve health, safety and independence. This level of medical care can facilitate a quick recovery, as well as prevent a medical condition from becoming a crisis. Nurses continuously coordinate with the primary physician and the patient to provide the best quality of care possible for each patient.
Home Health Aides
The home health aide assists with personal care needs of the patient, such as basic grooming for those who can no longer perform these tasks alone.
Medical Social Workers
The medical social worker offers comfort and support to patients and their family members as they encounter social and practical issues. This may include planning for the future, issues with coping, finances, or coordinating community resources.
Rehabilitation Therapy
Patients can receive their prescribed speech therapy, occupational therapy or physical therapy session in their own home. A therapist visit at home can help a patient live comfortably and safely in their own environment and avoid traveling for regular appointments.
