Here to Help
People facing health limitations often wonder, “What should I do if something happens?” A home health nurse is available by phone 24 hours a day, 7 days a week to answer questions, assess patient situations, offer support and put minds at ease. Having an expert available helps patients worry less and focus more on getting back on their feet.

Care at Home
A home health nurse meets with the patient on a regular basis to provide prescribed medical care, discuss changes in the patient’s condition and help resolve problems. While receiving this care, the patient is also educated about their condition and ways to remain safe, healthy and independent. This knowledge empowers the patient with tools to continue growing stronger and healthier, even after they have completed their goals for home health.

Support Services
Beyond the intermittent visits by the nurse, Optimal Home Health offers additional services such as rehabilitation therapy and assistance with day-to-day activities, such as dressing and bathing. Optimal also coordinates prescribed medications and medical equipment.
